To achieve high quality health care and improve patient saftey, shared care is a key construct. The Shared Care Programme is is seeking to explore a collaborative approach with all care providers supporting people with long term conditions. To enable this, the role of technology is foundational. During this presentation I will walk through the National Shared Care Programme and highlight the vision, the objectives and where we are at with regard to the anticipated benefits. |
